Infarction of papillary muscles and mitral insufficiency associated with congenital aortic stenosis.

چکیده
IN 21 INFANTS, each with symptomatic congenital aortic stenosis and left ventricular cavity of nearly normal size studied clinically,' evidence for mitral insuffiiegncy was frequently encountered. In five of the patients left ventriculography was done and, in each, mitral insufficiency was demonstrated. This background of experience led us to study, in particular, the mitral valve in specimens of heart from infants with congenital aortic stenosis. This study led to the conclusion that there is a common association between congenital aortic stenosis, on one hand, and infarction of the papillary muscles of the mitral valve, with resulting mitral insufficiency, on the other. It is the purpose of this report to describe the pathological features of this combination of lesions as a means of emphasizing its occurrence.

Double orifice mitral valve (DOMV) is an uncommon anomaly that was first described by Greenfield. Since then more than 200 cases have been reported. In this anomaly, mitral valve has a single fibrous annulus which opens with two openings into the left ventricle. We report a case of DOMV accompanied with severe stenosis in a 22-year old female who had complaints of moderate dyspnea. In transth...
متن کاملPacman Heart as a Congenital Cardiac Defect Associated with Flail Mitral Valve and a Partial form of Shone\'s Complex; A Case Report
Partial muscular inter-ventricular septal defect (VSD) or Pacman heart is a rare congenital or occasionally acquired anomaly. Concurrent Pacman heart and Shone's complex are extremely rare and have never been reported until now. We described a 37-year-old male patient with congenital Pacman heart, flail mitral valve (FMV), and a history of multiple congenital anomalies, including subvalvular ao...
متن کاملDiagnosis and management of the double orifice mitral valve: three case reports.
Double orifice mitral valve (DOMV) is an uncommon congenital heart defect. The isolated occurrence of this anomaly is very rare and, more often, is associated with another congenital malformation, dominated by atrioventricular canal defects (AVCD). Mitral insufficiency and/or stenosis may complicate this malformation. Treatment may be summarized as abstention, surgical valve repair, or valve re...
متن کاملCongenital mitral stenosis. Anatomical and functional assessment by echocardiography.
Digitised left ventricular echocardiograms were studied in nine children with congenital mitral stenosis to assess the severity of inflow obstruction. In six children the two prime indices of mitral stenosis were abnormal, with a prolonged time from minimum dimension to 20 per cent dimension change and a reduced peak dimension change during diastole. In three, however, these values did not sugg...
متن کاملTwo-dimensional echocardiographic examination of mitral valve abnormalities associated with coarctation of the aorta.
Coarctation of the aorta is frequently associated with left ventricular inflow tract abnormalities that may be difficult to detect even at cardiac catheterization. This study involved patients with coarctation who underwent comprehensive two-dimensional echocardiographic investigations emphasizing visualization of the mitral valve and its apparatus. Of the 56 patients studied, 23 had completely...
